First of all you need to turn it on in Appearance->Theme Options->Advertisement->Custom Banner X (X means a number of custom banner) as you can see on the picture below:

Then simply go to Appearance->Widgets and simply add J&W Custom Banner Widget element as you can see on the picture below (do not forget to set the right custom banner in the elements edit):
