Is there something already built into the store to add customer fields for Customer data?  For instance after customer address and billing address is there a way to add special custom fields.  For instance a customer questionnaire or something similar to that so I can use it to find data on the customers?  If it is not built in, is there a plugin that you can recommend getting that would get me something like this?
