Urgent: checkout page reloads after final step

Something very strange is happening in our site, which is why I had to take it offline. When a new customer goes through the checkout process on https://www.houseofliquid.nl/checkout/ at the end of the process, the form simple reloads. I can see that the system has created a new customer and logged the new customer in, but then the customer has to fill in all the steps in the form again.The system also doesn't save the customer data which was input (such as address, phone number).I've been trying to find out what the problem is but I just can't get my head around it :-( Could you check it out to see what we're doing wrong?
